EDUCATIONAL SERVICES
The Children's Central Intake Unit is the gateway into our programs serving children between the ages of 3-17. The intake process begins with referrals via telephone or face-to-face contact and is accomplished by a comprehensive initial evaluation. Consumers and families are then referred to an appropriate service within the agency or to an outside agency.
The Adult Intake Unit is the entry point into adult outpatient services where an initial intake assessment is performed and followed with an evaluation provided by Masters level clinicians and licensed psychiatrists respectively. Adults can also be admitted into various programs by referral from DBH or CBH.
The Educational Services Division's Mission is to provide a safe and structured educational environment where children and adolescents can be nurtured. We bring passion and commitment to the children in our care and understand and appreciate their differences and challenges. We recognize the "diamond in the rough" and seek to resuscitate the resilient spirit within.
While receiving services at Community Council Health Systems:

1.You have the right to be treated with dignity and respect.

2.You have the right to participate in the development and review of your treatment plan.

3.If you are taking medication, you have the right to know the name of the medication you are taking, why you aretaking it and what its possible side effects are.

4.You have the right to examine your Treatment Plan and record with your therapist.

5.You have the right to have all records at our facilities and anything you say here, remain completely confidential, subject to court order.

6.You have the right to have information about you released to other agencies only by your written permission authorizing a release of specific information.

7.You have the right to ask questions, file a grievance about your treatment in accordance with the posted grievance procedure and according to DPW regulations. At no time will a formal complaint have any negative repercussions on you during your course of treatment at our center.

8.You have the right to contact The Office of Civil Rights or The Pennsylvania Human Relations Department about any complaints you may have regarding discrimination.
